BW Schützt! Trauma rehabilitation for refugees

The BW Schützt! project is developing and testing a stepped care trauma rehabilitation program to enable early identification of psychological stressors that refugees may experience, so that appropriate counseling and support can be provided.

Background

Experiences in recent years confirm that a high number of refugees have experienced massive distress before, during and after their flight and consequently suffer from health problems and limited functionality in everyday life. This places a considerable burden on the care and integration of refugees. It not only requires efficient trauma rehabilitation after flight and violence for refugees living in Baden-Württemberg, but also preventive measures to hinder the development of debilitating mental disorders in refugees.

Although tested and proven effective treatment and prevention measures exist to alleviate the suffering and improve the functionality of those affected, these are rarely used due to various (linguistic, cultural and organizational) barriers.

Early identification of refugees with mental health problems and the provision of accompanying support should reduce or prevent functional impairments in everyday life associated with suffering, obstacles to integration, overburdening of refugee social work staff and high costs.

Objectives and target groups

The aim of the project is to develop and evaluate a program for the state-wide identification of psychological stress intensity among refugees in temporary accommodation and the provision of counseling measures for the subgroup of stressed persons. For this purpose, coaches are trained in screening and counseling. The coaches are culturally sensitive individuals with a migration background and language skills in the refugees' native languages as well as excellent social skills.

The long-term goal of the project is a well-founded, evidence-based recommendation for possible transfer to infrastructure measures in the state of Baden-Württemberg. The pilot project will be carried out in Karlsruhe and Konstanz and is scientifically evaluated in terms of feasibility and efficiency.

Components and procedure

Phase 1: Development of a counseling module on narrative biography work, training of coaches in screening and counseling as well as coordination of the accompanying study.

Coaches are trained in the implementation of all program steps (health interviews, counselling, preventative measures). The training and supervision will be provided by experts in the field of trauma rehabilitation.

Phase 2: Conducting health interviews to identify the intensity of symptoms and the respective need for care. Evaluation of the results and advice differentiated according to intensity of the symptoms. The symptom severity levels are indicated by "traffic light colors":

  • Green: no to low stress: careful monitoring with a focus on social and behavioral functionality; information about possible support services
  • Yellow: medium stress: biography-oriented narration to reduce stress, activation of resources
  • Red: high stress, high probability of (chronic) trauma-related disorders that cause considerable suffering and functional impairment; the measures offered here are: qualified clinical diagnostic interviews to confirm a diagnosis, treatment in line with guidelines according to the disorder: referral to psychosocial centers, cooperating private practices, therapists, outpatient clinics and hospitals as well as referral to outpatient or inpatient care in the event of self-endangerment or endangerment of others

The advice offered by the coaches should primarily be provided on site in the accommodation facilities. If necessary, trained language mediators are also available.

Phase 3: In accordance with the differentiated support offer, the coaches carry out narrative biography work to strengthen self-efficacy in cases of medium stress intensity.

Phase 4: A scientific evaluation accompanies the health interviews as well as the counseling and coaching measures in order to examine the effectiveness and efficiency and to enable quality control and management through continuous data collection. This forms an additional basis for the planned expansion of psychotherapeutic care for refugees in Baden-Württemberg.

Categories of data procession

We collect your name, your contact details and the data collected in questionnaires. Your details in the questionnaires are stored under a number (pseudonymization), and only authorized persons have access to a list linking the number to the name and contact details.

Purpose of data processing and consequences of not providing personal data

  • Participation and implementation of the research project

The Development Research Group at the University of Konstanz is developing and scientifically testing a program for low-threshold, differentiated care in order to identify psychological distress in refugees at an early stage and to offer those affected appropriate advice and support in the future.

This also includes collecting your contact details (telephone number or email address) in order to be able to contact you for the interviews and to invite you to the accompanying online survey. This contact data is stored separately from your details in the interviews and surveys. Contact details and information within questionnaires are required for the implementation of the "BW schützt!" research project; participation in the research project is not possible without this information. There are no disadvantages in the event of non-participation.

  • With appropriate consent

If you wish so, we will collect your e-mail address or telephone number in order to send you vouchers as a thank you for your participation in the accompanying study.

In the event that the above additional consent is not given, there will be no disadvantages.

Legal basis

  1. Implementation of the research project Art. 6 section 1 lit. e in conjunction with Art. 6 section 3 of the Datenschutz-Grundverordnung (DS- GVO) in conjunction with § 13 section 1 of the Landesdatenschutzgesetz Baden-Württemberg
  2. The basic willingness to participate in the research project and with consent for further optional uses Art. 6 section 1 lit. a) and Art. 9 section 2 lit. a) Datenschutz-Grundverordnung (DS- GVO) (consent).

Recipients

  • The data collected will be made available to the Development Research Group at the Department of Politics and Public Administration at the University of Konstanz and to the coaches for their consulting activities.
  • The publication of research results in reports or at conferences takes place exclusively in anonymized form and will at no time allow to draw conclusions about you as a person.
  • The external recipient is the service provider Qualtrics, which processes the data on our behalf and with whom an agreement on order processing has been concluded.

In addition, documents must be offered to the university archives in accordance with archiving regulations before they are deleted. The archive decides on the acceptance of documents.

Duration of data storage

  • All research data will be stored for ten years after completion of the project. The lists of codes for the assignment of persons are deleted one year after the end of the project.

If necessary, the documents will be taken over by the responsible university archive and stored there indefinitely.

Right to object

You have the right to object of processing your data based on Art. 6 section 1 lit. e) DSGVO in accordance with Art. 21 DSGVO. We will no longer process your data following an objection unless we can demonstrate compelling legitimate grounds for the processing.

Your other rights

Categories of data procession

We collect your name, your contact details and the data collected in questionnaires. Your details in the questionnaires are stored under a number (pseudonymization), and only authorized persons have access to a list with the assignment of number to name and contact details.

In addition to the data evidently collected in the questionnaire, audio recordings are made in the focus group discussions. These are then transcribed, i.e. written down according to certain rules. Furthermore, the audio recordings are annotated with metadata (place, time).

The Development Research Group at the Department of Politics and Public Administration at the University of Konstanz is developing and scientifically testing a program for low-threshold, gradual help in order to identify psychological stress in refugees at an early stage and to offer those affected appropriate advice and support in the future. The data collected as part of the research project from the online surveys and focus group discussions has the goal to evaluate your motivation, experiences and expectations of your work as a coach in the "BW schützt!" project.

Your contact details (telephone number or email address) are required in order to invite you to the online surveys. This contact data is stored separately from your details in the surveys. Contact details, information within questionnaires, audio recordings and transcripts are required for the implementation of the "BW schützt!" research project; participation in the research project is not possible without this information. There are no disadvantages in the event of non-participation.
